The first few lines do not include the move from D to D/C but stay on the D for two measures, and then the chord progression as written above is correct. But later in the song the verses have a D - D/C - D/B - D/A# progression, and toward the end there is a modulation where the A - A/G - G - Em structure becomes Bm - Bm/A - G - Em.
